If you are seeking coverage for facial or body hair removal through Gender Affirming Care and PCOS, hirsutism, hypertrichosis and follicular disorders, please ensure that you meet the requirements outlined To initiate the process, you'll need to submit the following documentation:
1. Letters of Support: - A letter from the provider managing your gender-affirming hormone therapy (within the past 18 months). - A letter of medical necessity from the treating provider (dermatologist or primary care provider) completed in the last 18 months. This letter should detail the hair removal techniques attempted, any failures by body region, and the medical condition preventing the use of other hair removal methods. (Note: The letters may come from the same provider, presented in letter format with all required information.)
2. Photo Documentation: - Before hair removal photos: Face and neck (8-12 hours of hair growth recommended); Body (1-2 weeks of hair growth for visibility). - Immediately after hair removal photos: Capture the area post-removal, considering an overnight hair regrowth period.
NO INTIMATE AREA PHOTOS REQUIRED OR REQUESTED FOR INSURANCE COVERAGE.
3. Provider's Letter for Hair Removal: - A letter of medical necessity from the provider who will perform the hair removal. This should specify the size and location of the treatment area and the expected number of units needed per body region for the requested treatment.
4. Body Regions for Treatment: - Face/neck, Chest, Abdomen, Axillary (underarm), Arm, Upper back, Lower back, Buttocks, Inner thigh (swimsuit/bikini area), Genital area, Upper leg, Lower leg. By adhering to these requirements, you can streamline the process and enhance the likelihood of successful insurance coverage for your electrolysis treatments.
